1002 Bilateral Nonarteritic Anterior Ischemic Optic Neuropathy (NAION) in a Patient with Obstructive Sleep Apnea (OSA)

Abstract Introduction Nonarteritic Anterior Ischemic Optic Neuropathy (NAION) is an ischemic insult to the anterior portion of optic nerve with unclear etiology. Several small sized studies and case reports have demonstrated association between OSA NAION. In this case, we present a patient bilateral NAION who was later diagnosed which may signify important clinical association. Report case(s) A 57-year-old man history hypertension, presented ophthalmology clinic for sudden painless decreased visual acuity right eye one-month duration. He had no head trauma or other neurological deficits on his initial presentation. reported having “bad vision” in left entire life. Funduscopic evaluation revealed disc edema more eye. Laboratory workup including CBC, CMP HbA1C were within normal limits. Brain MRI showed only mild nonspecific scattered subcortical deep periventricular hyperintensities suggestive chronic vessel disease acute infarct masses. orbits normal. diagnosis highly suggested based examination findings. referral sleep made evaluate possible apnea. polysomnogram confirmed obstructive apnea AHI 7.2/h, REM 7.7/h supine 14.9/h. Nocturnal hypoxemia associated respiratory events time O2 saturation < 88% 29 minutes. Patient referred titration study determine appropriate PAP pressure treat disordered breathing. Conclusion This adds current evolving evidence link It signifies importance index suspicion, among both specialists ophthalmologists, recognize patients one conditions consider screening other, lead earlier improved outcomes.
